Extra Support When Older Adults are in a Tulsa Hillcrest Hospital

Hillcrest offers complimentary extra support for senior patients and their family members while in Hillcrest Medical Center, Hillcrest South and Hillcrest Claremore. 

Discharge Planning

If your loved one is hospitalized or has recently been in the hospital, please read Your Guide to Discharge Planning. It will give you some tips on transitioning from the hospital and planning for care needs after discharge.

If your loved one was recently discharged from Hillcrest Medical Center or Hillcrest South, feel free to call our patient care specialists at Hillcrest Medical Center (918-579-8040) or Hillcrest South (918-294-4818) if you need help.

FREE Medication Assessments

Do you or a loved one have concerns about medications? Hillcrest offers FREE medication assessments for seniors age 60+. A clinical pharmacist will review your or your loved one’s medications for free. Call 918-579-8040 to schedule an appointment at the location nearest you. Locations include Midtown Tulsa, South Tulsa and Owasso.

Medication assessments are by appointment. Please provide a list of medications in advance if possible, and bring the medications (in their original containers) to the appointment. Include over-the-counter medications, vitamins, supplements, and herbal remedies such as teas.

Adult Day Programs

Adult day programs provide caregivers the worry-free opportunity to work, run errands or take a deep, relaxing breath, while loved ones spend their days filled with friends, laughter and enriching activities. LIFE's Adult Day Health Centers provide care for seniors and other adults in a safe, friendly and supportive setting, enabling family caregivers to find a healthy balance between work, home and the responsibilities of being a caregiver. Adult Day Health programs are proven to be a cost-effective care option that delay or even avoid altogether the need for nursing home placement. Medicare Wellness Visits

Utica Park Clinic’s Medicare exam page has helpful information about Medicare wellness visits. For those with Medicare, these visits are encouraged within 12 months of Medicare enrollment and annually after that. There is no cost to the Medicare beneficiary.
